ADULT VERIFICATION FORM:
Every adult participant in GA KidMin events (18 & older) MUST fill out an Adult Verification form. Please print this form for each adult leader, have them complete the form, then get it signed & approved by the Lead Pastor (or if the Lead Pastor is unavailable- a board member).
Forms should be scanned (not photographed) & emailed to Liz Reneau (elizabeth@gadistag.org) or mailed to
GA KidMin ATTN: Liz Reneau PO Box 28470 Macon GA 31221.
Any adult who does not have this form approved and signed will not be allowed to participate in any district function for GA KidMin. You will only need to complete one form per calendar year.
